We are committed to your comfort and privacy during the process of therapy. We want you to feel completely relaxed in knowing that your sessions are confidential and private at all times. Only at your request and with your written permission would we ever discuss your involvement in therapy with another person.


24-hour Cancellation Policy: In order to provide a convenient schedule for all of our clients, if you cancel a booked appointment with less than 24-hours notice, full payment will be due. Thank you for your understanding of this important policy.

New Client Forms

If you are a new client,  print and complete the following paperwork prior to your first appointment & email back to us with a copy of your insurance card.  You may also bring this paperwork with you to your first appointment.
